MECHANICS, TERMS & CONDITIONS
1. How to avail of the Cordlife Privilege for FSP Members?
Sample Collection Process
• Call the Cordlife hotline at 332-1888 or e-mail info.ph@cordlife.com in availing of the Plumcare DNA Advisor (Trio Package - Father, Mother & Child).
• Collection kit will be delivered to the member’s location preference (within or even outside Metro Manila)
• Sample collection will be shipped to their USA laboratory for analyzation.
• Results will be e-mailed directly to the client after 16 weeks.
Payment Options
• If situated within Metro Manila and will pay at the time of sample collection- FSP member shall present his/her FSP card/mobile app as proof of membership
• If situated within Metro Manila and will pay through bank deposit- FSP Members shall send a scanned copy of FSP Card or screenshot of FSP Mobile App as proof of membership*
• If situated outside Metro Manila will have to pay through bank deposit- FSP Members shall send a scanned copy of FSP Card or screenshot of FSP Mobile App as proof of membership*

2. For referral of Pregnant Mothers
• Access the link via www.cordlife.ph/RAF and accomplish the required information details.
• A Cordlife representative will call the member to verify details. Upon successful referral, Cordlife will automatically send the Rustan’s gift certificates to the member 30-45 days after.
3. How to avail of the FSP Privilege for Cordlife Privilege Cardholders?
• Privilege card must be presented to any of the Rustan’s Customer Service area for the following:
• 10,000 FSP Bonus (Manual crediting through card application)
• Free FSP Card if availing of the FSP Membership
• Free Access to Rustan’s Premium Lounge
